GENERAL INFORMATION

First Prize winners in each market will be notified by email, mail and/or phone on or about the day after appearance of the OPTIMA Bass Pro at the Bass Pro Shop. Grand prize winner will be required to sign an Affidavit of Eligibility and Liability/Publicity Release and return it within seven (7) days of attempted delivery. Prize will be awarded to an alternate winner if Affidavit is not so returned or prize notification is returned as undeliverable. An IRS Form 1099 will be prepared accordingly, if applicable. Winners hereby consent to the use of their names, voices and/or likenesses by the Sponsor and its partners for advertising purposes without additional compensation unless otherwise prohibited by law. Winner is responsible for all federal, state and local taxes. By entering, participants agree to be bound by the Official Rules and the decisions of the judges, which are final in all respects. By participating in the promotion, participants and winners release Sponsor and their parents, partners, affiliates, subsidiaries, officers, agents, employees, advertising agencies and all others associated with the development and execution of the Sweepstakes from and against any and all liability with respect to or in any way arising from the Sweepstakes. By claiming a prize, winner agrees that all prizes are awarded upon the condition that Johnson Controls, Inc., and its respective agencies or employees, will have no liability whatsoever for any injuries, losses or damages of any kind caused by any prize or resulting from the acceptance, possession or use of any prize. Prizes are not transferable and not redeemable for cash. Sweepstakes is void in Puerto Rico and where prohibited by law. All federal, state and local laws apply.
