Sweepstakes begins on March 26, 2011 and ends on June 4, 2011. Sweepstakes is open to all legal residents of the continental U.S. who are 21 years of age or older at time of entry. The sweepstakes is sponsored by Johnson Controls, Inc., Milwaukee, WI.
TO ENTER: Fill out an Official Entry Form found on displays at participating Bass Pro stores nationwide and put it into the provided entry box on the day the OPTIMA Bass Pro is visiting the Bass Pro stores in selected markets. One entry per person per day. Each Official Entry Form must be original; no photocopies or other reproduction of forms or entries permitted. All entries become the property of the Sponsor and will not be returned. Sponsor and participating stores are not responsible for lost, damaged, incomplete or illegible entries.
First Prize winners will be selected, in a random drawing, from all eligible entries received following the visit of the OPTIMA Bass Pro. Winner does not have to be present to win. Grand Prize winner will be determined in a random drawing between the 8 First Prize winners. Odds of winning a First Prize will be determined by the number of eligible entries received. Approximate number of entries to be distributed is 16,000. No substitution of prize is permitted by winner.
PRIZES: One (1) Grand Prize - A one-day, two-night outing to fish with BASS PRO Edwin Evers on a lake near his home in Tulsa, Oklahoma. Prize includes airfare from the nearest major airport to the winner, double occupancy hotel, and $200 cash for incidentals. No cash alternative of the prize is offered but if the prize becomes unavailable for any reason, the Sponsor may substitute a prize of equal or greater value at the Sponsor's discretion. Approx. Retail Value (ARV) $1,500.00. Actual prize value may vary. Any expenses not mentioned are the sole responsibility of the winner. If the announced winner cannot attend the trip before July 2012, then he/she will forfeit the prize and an alternative winner will be selected if time allows. Eight (8) First Prizes-A gift certificate that can be redeemed for a free OPTIMA battery of the winner's choice. ARV $200.00. ARV of all prizes is $3,100.00.
ELIGIBILITY: Promotion open to all legal residents of the continental U.S. who are 21 years of age or older at time of entry, except employees and their immediate families (spouses and parents, siblings and children and their spouses) of Bass Pro Shops and the Sponsor, their affiliates or subsidiaries, their advertising agencies and any other company involved with the design, production, execution or distribution of this promotion.
GENERAL: First Prize winners in each market will be notified by email, mail and/or phone on or about the day after appearance of the OPTIMA Bass Pro at the Bass Pro Shop. Grand prize winner will be required to sign an Affidavit of Eligibility and Liability/Publicity Release and return it within seven (7) days of attempted delivery. Prize will be awarded to an alternate winner if Affidavit is not so returned or prize notification is returned as undeliverable. An IRS Form 1099 will be prepared accordingly, if applicable. Winners hereby consent to the use of their names, voices and/or likenesses by the Sponsor and its partners for advertising purposes without additional compensation unless otherwise prohibited by law. Winner is responsible for all federal, state and local taxes. By entering, participants agree to be bound by the Official Rules and the decisions of the judges, which are final in all respects. By participating in the promotion, participants and winners release Sponsor and their parents, partners, affiliates, subsidiaries, officers, agents, employees, advertising agencies and all others associated with the development and execution of the Sweepstakes from and against any and all liability with respect to or in any way arising from the Sweepstakes. By claiming a prize, winner agrees that all prizes are awarded upon the condition that Johnson Controls, Inc., and its respective agencies or employees, will have no liability whatsoever for any injuries, losses or damages of any kind caused by any prize or resulting from the acceptance, possession or use of any prize. Prizes are not transferable and not redeemable for cash. Sweepstakes is void in Puerto Rico and where prohibited by law. All federal, state and local laws apply.
WINNERS LIST: The First Prize winner in each market will be posted on the OPTIMA Battery website at www.optimabatteries.com approximately one week after each appearance by the OPTIMA Bass Pro Shop. The Grand Prize winner will be posted on or about September 1, 2011.
